Does Your Child Have a Life-Threatening Health Condition?

To safeguard students who have life-threatening medical conditions such as severe food allergies, CDNIS has two important forms for parents to fill out:

  1. Permission for Medication form. This form is permit school nurses to hold and administer associated medication to students with life-threatening conditions.
  2. Emergency Action Plan form. This form details the symptoms, precautions, and steps CDNIS should take on behalf of a student in case of emergency associated with a life-threatening condition.

New and returning students need to fill out these forms every year to ensure that all important information is always current and accurate. Please contact the school nurse with any questions you may have about these forms and protocols at
